Areas of support may include:
- Developing anxiety management strategies
- Building coping skills for stress and overwhelming emotions
- Establishing healthy daily routines
- Improving confidence and social engagement
- Developing self-management techniques for low mood
- Supporting wellbeing and resilience
- Signposting and connecting patients with appropriate community services and support networks
The service promotes independence and self-management using a recovery-focused approach, helping patients build the skills and confidence needed to manage their mental health and wellbeing.
Escalation Pathway
Where a patient’s needs are more complex, or their mental health deteriorates during support, they can be escalated to the PCN Mental Health Clinician, for further assessment and intervention (up to six sessions where appropriate).
