Proxy Access for Children’s medical record under 11 Years old

For Children’s medical record under 11 Years old- For parents access to childs records

To obtain proxy access the parent will need to be registered for online access at our practice.

Parent with proxy access will be able to book appointments and order repeat medications for the child, and will also have access to the elements of the childs record that have been released in the for online access.

The parent must show proof of ID such as a passport/driving licence and proof of parental right eg-Birth Certificate at the time of requesting proxy access for the child. If there are any limitations on access to the child or their information imposed by a Court or Social Services this must be declared at this point.
