For Adults Only who require Proxy Access for Family Member/Carer/Legal Representative
To obtain proxy access the user will need to be registered for online access at our practice.
Family Member/Carer/Legal Representative with proxy access will be able to book appointments and order repeat medications for the patient, and will also have access to the elements of the patients record that have been released in the for online access.
The Family member/carer/legal representative must show proof of ID such as a Driving Licence/Passport and proof of address for themselves. If there are any limitations on access to the patient or their information imposed by a Court or Social Services this must be declared at this point.
